چکیده

INTRODUCTION The introduction of total mesorectal excision (TME) and neoadjuvant chemoradiotherapy have further promoted pelvic MRI as the staging procedure of choice in rectal cancer [1]. Circumferential resection margin (CRM), local T-stage and nodal stage are the most important factors influencing therapy and outcome, but the optimal imaging protocol to accurately assess these image features is yet under debate [2]. Although the necessity to perform high-resolution T2-weighted imaging is commonly accepted, clinical data with an image slice thickness of less than 3mm is not yet available. In this study we therefore sought to evaluate thin-slice, highresolution pelvic MRI with 3D sequences for rectal cancer staging with a focus on detection of nodal disease and the diagnostic value of contrast enhanced imaging.

MR imaging for preoperative evaluation of primary rectal cancer: practical considerations.

High-resolution magnetic resonance (MR) imaging plays a pivotal role in the pretreatment assessment of primary rectal cancer. The success of this technique depends on obtaining good-quality high-resolution T2-weighted images of the primary tumor; the mesorectal fascia, peritoneal reflection, and other pelvic viscera; and superior rectal and pelvic sidewall lymph nodes. Although orthogonal axial...

Magnetic resonance imaging for diagnosis and neoadjuvant treatment evaluation in locally advanced rectal cancer: A pictorial review

High-resolution pelvic magnetic resonance imaging (MRI) is the primary method for staging rectal cancer. MRI is highly accurate in the primary staging of rectal cancer; however, it has not proven to be effective in re-staging, especially in complete response evaluation after neoadjuvant therapy. Neoadjuvant chemoradiotherapy produces many changes in rectal tumors and on adjacent area, as a resu...

The accuracy of High Resolution Magnetic Resonance in local staging of rectal cancer

Objective: To assess the accuracy of preoperative staging of rectal cancer with magnetic resonance imaging (MRI) and illustrate the correlation of MR imaging with pathologic findings and clinical impact of MR imaging in this setting. Methods: Total of 447 patients with rectal cancer between 01.04.2009-31.03.2010 underwent MRI in our department. 76 patients with suspected or confirmed rectal car...

The importance of rectal cancer MRI protocols on iInterpretation accuracy

BACKGROUND Magnetic resonance imaging (MRI) is used for preoperative local staging in patients with rectal cancer. Our aim was to retrospectively study the effects of the imaging protocol on the staging accuracy. PATIENTS AND METHODS MR-examinations of 37 patients with locally advanced disease were divided into two groups; compliant and noncompliant, based on the imaging protocol, without kno...

MRI in local staging of rectal cancer: an update.

Preoperative imaging for staging of rectal cancer has become an important aspect of current approach to rectal cancer management, because it helps to select suitable patients for neoadjuvant chemoradiotherapy and determine the appropriate surgical technique. Imaging modalities such as endoscopic ultrasonography, computed tomography, and magnetic resonance imaging (MRI) play an important role in...
